Individuals who seek records of prior claims related to work injuries or open records may complete and submit the appropriate forms below based on the type of record sought. Please note that certain fees may apply based on our Schedule of Fees.

For all other claims and copy information not found in the CaseOK system, please complete a Request for Prior Claims and Copy Request Form. Completed forms or questions about this form may be emailed to Records@wcc.ok.gov.

Open Records

The purpose of the Oklahoma Open Records Act is “to ensure and facilitate the public’s right of access to and review of government records so they may efficiently and intelligently exercise their inherent political power.” 51 O.S. § 24A.2.

Individuals wishing to submit an Open Records Act request should fill out the Open Records Request form. Please do not use this form to request records related to your case. Requests of that nature should be submitted as a Prior Claim File request - See the Section above.
